Diabetic and Nutrition Therapy

Being diagnosed with diabetes can feel overwhelming. At Deckerville Community Hospital, our team of certified diabetes educators works with you to teach the skills needed to manage diabetes – whether you have been newly diagnosed or whether you have had diabetes for years.

Diabetes Self-Management Training Program: Diabetes education can help prevent, delay or reverse complications through effective self-management.

  • Individual Counseling provides instruction in diabetes self-management. This can occur individually, in groups, or a combination of the two.
  • Dietician services are available to help each patient develop an individualized meal plan. A registered dietician will provide nutrition therapy.
  • Group Classes are provided in a series of four (4) sessions to help you better understand diabetes and to learn to manage your diabetes by making positive changes in your life.

Topics Include:

  • What is diabetes?
  • How activity affects your diabetes
  • Meal planning and food choices
  • Prevention of long-term complications
  • Stress management
  • Community resources

Follow-up Sessions offer ongoing support and encouragement.  

The Diabetes Education Program at Deckerville Community Hospital is accredited by the American Diabetes Association and the Michigan Department of Community Health.

Diabetes Education is a covered benefit of most health insurances for individuals with a diagnosis of diabetes, including Medicare and Medicaid. A physician referral is required, and we will be happy to assist with this.
