Patient Rights and Responsibilities

The following are the basic rights and of patients at DCH:

  • The patient has the right to informed participation in decisions involving his/her health care. The patient shall not be subjected to any procedure without voluntary, competent and understanding consent, or that of the patient's legal authorized representative. The patient has the right to refuse medical treatment including respirators, medically provided nutrition/hydration and antibiotics.
  • The patient has the right to obtain for the practitioner responsible for coordinating care, complete and current information concerning the diagnosis (to the degree known), treatment, and any known prognosis.
  • Individuals shall be accorded impartial access to treatment or accomodations given in a responsible, considerate and respectful manner.
  • The patient has the right, within the law, to personal and informational privacy and confidentiality.

  • The patient is entitled to recieve adequate and appropriate care.
  • The patient has the right to expect a reasonably safe and secure environment insofar as the hospital practices and environment are concerned.
  • The patient has the right to know the identity and professional status of individuals providing service to him/her and to know which physician or other practitioner is primarily responsible for his/her care.
  • The patient is entitled to information about the hospital rules and regulations affecting patient care and conduct.
  • The patient has the right of access to or refusal of people outside the hospital by means of visitors and by verbal and written communication. When the patient does not speak or understand the predominant language, every effort will be made to secure and interpreter.
  • Regardless of the source of payment for care, the patient has the right to request and recieve an itemized and detailed explanation of the total bill for services rendered in the hospital. The patient has the right to timely notice prior to termination of eligibility for reimbursement by any third-party payer for the cost of care. The patient has the right to recieve upon request information relating to financial assistance available through the facility.
  • The patient, at their own request and expense, has the right to consult with a specialist.
  • The patient is entitled to be free from mental and physical abuse and from physical and chemical restraints except those restraints authorized in writing by the attending physician for a specified and limited time or as are necessitated by an emergency to protect the patient from injury to self or others.
  • A patient is entitled to present grievances or reccomend changes in policies and services on behalf of self to the facility staff, or to another pseron of the patient's choice.
  • When conflicts or ethical issues arise involving a patient, anyone concerned with the situation may request a meeting of the Ethics Review Team.

Pediatric Rights:

  • Involvement of family and/or guardian through course of treatment.
  • When a conflict arises concerning the care of the patient, the guardian has the right to initiate a meeting of the hospital's Ethics Review Team.
  • Recieve assistance to family in coping with particularily traumatic or lengthy illness.
  • Have provision for activities of daily living appropriate to age.
  • Availability of appropriate furniture and equipment for child's age, size, and development needs.

Patient Responsibilities:

  • The patient has the responsibility to provide, to the best of knowledge, accurate and complete information about present complaints, past illnesses, hospitalizations, medications and other matters relating to their health.
  • The patient is responsible for following the treatment plan recommended by the practitioner primarily responsible for their care.
  • The patient is responsible for his/her actions if treatment is refused or the patient does not follow the practitioner's instructions.
  • The patient is responsible for assuring that the financial obligations of their health care are fulfilled as promptly as possible.
  • The patient is responsible for the following hospital rules and regulations affecting patient care and conduct.
  • The patient is responsible for being considerate of the rights and property of other patients and hospital personnel.

If You Have A Medical Emergency Or Are In Labor:

You have the right to recieve within the capabilities of this hospital's staff and facilities:

  • An appropriate medical screening evaluation;
  • Necessary stabilizing treatment (including treatment for an unborn child) and if necessary,
  • An appropriate transfer to another facility even if you cannot pay or do not have medical insurance or you are entitled to Medicare or Medicaid.

 

This hospital does participate in the Medicaid program.
