Explanation of Measure Importance Reason for Transfer: Documentation of the reason(s) for the transfer to help ensure that proper transfers have occured according to the current guidelines. Receiving Hospital Accepted the Patient: Documentation that the physician at the receiving hospital has accepted the patient to be transferred to help to ensure responsibility for evaluation and treatment according to current regulations. Physician Accepted the Patient: Documentation of the physician at the receiving hospital has accepted the patient to be transferred to help ensure responsibility for evaluation and treatment has occurred according to current regulations. Patient's Benefits and Risks for Transfer: Documentation that patient and/or significant other are provided information that identifies the risk and benefit of the transfer to another facility according to current regulations. Patient's Stability: Documentation of the patient's stability, including blood pressure and pulse rate (BP/P/R), is documented on the patient medical record for each patient transferred. Vital Signs Taken and Recorded within 15 Minutes of Transfer: Documentation of patient's vital signs including blood pressure and pulse rate (BP/P/R) prior to release of patient for transfer within 15 minutes of transfer as required by current regulations. |
