Bassam Afaneh earned his Medical Doctor degree from the University of Cairo in 1975. After practicing many years in his home country of Jordan, he went to Ireland in 1985 to study pediatrics. He came to the United States in 1990 to complete his residency in Pediatrics at Michigan State University-Hurley Medical Center in 1992, where he was appointed instructor in the Pediatric Department. In 1994, he joined a pediatric practice in Nablus-West Bank, Jordan, but then returned to the USA to join a private practice in Michigan. Dr. Afaneh joined the Medical Staff of Deckerville Community Hospital in 2001.

At Deckerville Healthcare Services, Dr. Afaneh practices pediatrics, treating patients from newborn through 18 years of age and upward – for all types of medical needs. Dr. Afaneh specializes in the diagnosis and treatment of asthma and otitis media (middle ear infection), including the prevention of hearing loss that is associated with both these conditions.

Dr. DeCossard joined the Deckerville Community Hospital staff in 2004, after receiving his Geriatric Medicine Fellowship from Loyola University Health Systems / Hines VA Hospital in Illinois. He lived for several years in the "Windy City" of Chicago, and then moved here to our rural community, where he has made his home with his wife Michelle (who is also a physician) and his two young daughters, Regine and Jasmine. Dr. DeCossard has a special interest in memory problems caused by Alzheimer’s disease and other types of dementia. Over the course of his career, Dr. DeCossard has worked as a physician in several hospitals, focusing his philosophy on what he terms a "reverence for life", and using this basic principle to guide his medical practice.


Levi Guerrero, MD • Chief of Medical Staff

Dr. Guerrero, MD, attained his Bachelor of Science at Xavier University, Philippines and Doctor of Medicine at the University of Santo Tomas, Manila, Philippines in 1970, then worked as a staff physician at Bukidnon Hospital until 1975.

Dr. Guerrero and his wife Ophelia (Ofie) came to the United States in 1975. Dr. Guerrero did his first year pathology residency at St. Mary’s Hospital in Grand Rapids, completed his pathology residency at Detroit Macomb Hospital Association, and then worked in nuclear medicine at William Beaumont Hospital. He and his family moved to Deckerville in 1982, and Dr. Guerrero has been an active member of the Medical Staff of Deckerville Community Hospital since then. Dr. Guerrero and Ofie have three children- Manual "Manny", Michael and Trisha.

Steven Jensen, MD • Urologist

Dr. Steven Jensen is the founding partner of Tri City Urology. Dr. Jensen is a diplomat of the American Board of Urology and holds memberships in the American Medical Association, American Urologic Association, and the Michigan Stat Medical Society.

Dr. Jensen specializes in adult and pediatric urology, female urology, urologic oncology, and minimally invasive treatment of kidney stones. He is the only urologist in the Tri City Area trained and experienced in laparoscopic urologic surgery, including laparoscopic prostatectomies and nephrectomies.

Dr. Jensen is one of the few leaders in the state of Michigan with experience and training in neuromodulation of the bladder for urinary incontinence (InterStim/bladder pacemaker). He also has specialty training in robotic prostatectomy (daVinci) for prostate cancer.

Dr. Jensen incorporated cryotherapy into his arsenal of prostate/kidney cancer treatments because it has evolved into an equivalent treatment option for localized prostate/kidney cancer. Because of its minimally invasive nature, cryotherapy offers decreased morbidity (side effects) compared to surgery and radiation.

Dr. Jensen has specialty training in renal transplantation and vascular surgery. He also has extensive experience in office ultrasonography, cystoscopy, vasectomies, and microwave therapy for prostatic enlargement.

Gerald Jerry, MD • Orthopedic Surgeon

Dr. Jerry is a board certified orthopedic surgeon specializing in the management of arthritis as well as knee, hip and shoulder replacement. He has been in private practice in Port Huron for more than 20 years.

He received his medical degree from the State University of New York, Upstate Medical Center in Syracuse, New York. He served an intership at Brown University in Rhode Island, before completing a residency in Orthopedics at SUNY Upstate Medical Center, where he was chief resident for a year. He then went on to complete a one year fellowship in adult reconstruction at the Mayo Clinic in Rochester, Minnesota.

He is certified by the American Board of Orthopedic Surgery, and a member of the American Medical Association, Michigan State Medical Society, American Academy of Orthopedic Surgeons, American College of Physician Inventors, Mayo Clinic Fellow Association and SUNY Medical Alumni Association.

One of the world’s preeminent minimally invasive spine surgeons, Dr. Gerald R. Schell’s roots stem from the Thumb of Michigan.

Dr. Schell attended Olivet College in Olivet, Michigan and later received his Doctor of Medicine degree from Michigan State University’s College of Human Medicine before completing a neurosurgical rotation at Syracuse University. Syracuse University later offered him the directorship of its neurosurgical training program, which he declined to continue caring for the needs of Michigan patients.

Currently, Dr. Schell uses advanced minimally invasive surgical techniques for the vast majority of his patients with spinal disc disease. For patients who previously had no hope for pain relief for osteoporotic compression fractures, Dr. Schell performs novel procedures such as the three dimensional CT reconstructive techniques with balloon kyphoplasty.

Dr. Schell performed the first artificial lumbar disc surgery in the state of Michigan, and both he and Dr. Field are currently playing a leading international role in the experimental development of single-level and two-level artificial discs for cervical disk problems. They are also involved with numerous other collaborative scientific endeavors to help advance neurosurgical care. Additionally, Dr. Schell served as the Chief of Staff of St. Mary’s of Michigan hospital in Saginaw until 2009.
